AUTHORIZATION TO USE AND DISCLOSE PROTECTED HEALTH INFORMATION I authorize my healthcare providers, health plans, and pharmacies ("Healthcare Providers") to disclose my individually identifiable information related to my health, treatment, care, insurance coverage, payment information, and medication history ("Protected Health Information") to SouncaLink, LLC and its designated affiliates, agents, representatives, and service providers. This authorization allows SouncaLink, LLC to:

1. Enroll me in, provide, and operate the SouncaLink, LLC Medical Therapeutic and Diagnostic Units.
2. Develop, evaluate, and improve our Link and Arounca products and related services.

I understand that once my Protected Health Information is disclosed to SouncaLink, LLC, it may no longer be protected by HIPAA and may be subject to redisclosure.

I acknowledge that I am not required to sign this Authorization and that my decision will not affect my treatment, payment, health insurance enrollment, or eligibility for healthcare benefits. However, I understand that my participation in the SouncaLink, LLC Beta Program requires my authorization.

This Authorization will expire when I am no longer participating in the SouncaLink, LLC Beta Program unless I cancel it sooner.
